Delta Health Care regrets to inform you that, as of December 21, 2007, Family Planning Clinic services have been discontinued at our clinic located at 318 South Crescent Street in Lodi, CA due to economic reasons.
WIC services at the Lodi site will continue.
This closure does not affect the other Delta Health Care programs, including: Education and Outreach Programs, School-Based Health Programs, and the Women, Infants and Children (WIC) Nutrition Program.
We are continuing appointments for clients with pending follow-up through February 2008. All other clients are being referred to other local healthcare providers. Staff will continue to provide client charts to clients’ new healthcare providers upon receipt of a signed Authorization for Release form. Former Delta Health Care clients may call (209)370-6809 or 6810 for further information regarding release of medical records.
